About the Role
The Mental Health Professional II will work closely with the Social Work Department to provide clinically driven comprehensive discharge planning for mentally ill patients. This role requires strong collaboration and communication skills, as well as knowledge of benefit entitlements and community resources.
Develop and implement individualized treatment plans in collaboration with the multidisciplinary treatment team.
Provide education and support to patients and their families on mental health topics and community resources.
REQUIREMENTS:
Valid license as an LMSW or LCSW issued by the NYSED.
