Job Description
This is an exciting opportunity for an experienced Social Worker to support the opening of Building 8b, Wāhi Tautoko. The role involves working closely with a multidisciplinary team to strengthen therapy skills in mental health and provide support to consumers who require extended assessment or treatment following an admission to the acute inpatient service prior to discharge.
The goals of the service include providing a safe and supportive environment to provide extended assessment, treatment, and rehabilitation, teaching skills to help support illness management, and supporting consumers to live the life they want to live.
In this role, you will be responsible for completing thorough social histories, conducting assessments of consumers, and formulating interventions to meet individual needs in collaboration with the multi-disciplinary team (MDT). You will also support family/whānau engagement, work alongside consumers on complex areas including debt consolidation, welfare guardianship, and PPP&R matters, and support consumers with integration into the community and development of strong relationships with community providers.
Key Responsibilities:
Completing thorough social histories
Conducting assessments of consumers and formulating interventions to meet individual needs in collaboration with the MDT
Supporting family/whānau engagement
Working alongside consumers on complex areas
Supporting consumers with integration into the community
Requirements:
A minimum of 2 years' experience in a mental health setting is preferable
A current NZ driver's license is essential
New Zealand Registration as a Social Worker with Annual Practicing certificate is essential
