About the Role

The NDIS Care Coordinator is an entry-level role designed for individuals looking to build a career in the NDIS sector. You’ll work closely with experienced NDIS Support Coordinators to learn how to assist participants in navigating their NDIS plans, connecting with services, achieving their goals and building their capacity. This is a great opportunity to develop foundational knowledge of the NDIS while making a meaningful impact on the lives of our community members. Full training and mentorship will be provided to help you grow into a Support Coordinator role.

Duties

Support participants in understanding the basics of their NDIS plans and available supports.
Assist in linking participants with appropriate service providers, community resources, and culturally safe supports.
Learn to develop simple action plans in collaboration with the Support Coordinator, focusing on short-term goals.
Provide administrative support, including maintaining case notes and ensuring compliance with policies.
Observe and participate in meetings with participants, families, and stakeholders to gain hands-on experience.
Develop a strong understanding of culturally safe and person-centred approaches to service delivery.
Participate in training and mentoring opportunities to build skills for future career progression.

Desired Skills and Experience
Required

Certificate lV or higher qualifications in Health Care, Social Assistance, Community Services, Administration and/or one year disability/age care experience.
Demonstrated ability to record accurate and consistent case notes.
Demonstrated ability to work with a minimum of direction and to effectively communicate with and work collaboratively as part of a multi-disciplinary team, and to contribute to the development of creative and innovative services.
Strong organisational skills and ability to work independently and/or as part of a team.
Ability to communicate with sensitivity and effectively with Aboriginal and Torres Strait Islander people.
Sound knowledge of Microsoft Suite Applications.
Current Drivers Licence (Qld).
Current National Police Clearance or ability to obtain.
Working with Children’s Card (Blue Card) or be eligible to apply.
Disability Worker Screening (Yellow Card) or be eligible to apply.
Willing to travel to our other locations.
Desirable

Previous experience of working with Aboriginal & Torres Strait Islander clients.
Previous experience in working with the National Disability Insurance Scheme (NDIS).
Previous experience in case management and coordination activities using a client information system.
Demonstrated ability to meet targets and performance outcomes.
COVID-1+ Requirement

Immunisation history showing you have received two doses of a Covid-1+ vaccine is encouraged.
