At Advance Support Co Pty Ltd we are passionate about providing exceptional support coordination services to our clients. We currently looking for a super star support coordinator to join our team.
You will be working with a manageable caseload of lovely NDIS participants to support them to build capacity and implement supports to help them achieve their goals outlined in the plans. You will be great at rapport building and have excellent knowledge of NDIS systems.
You will be joining a highly experienced and supportive team. We are looking for someone who can hit the ground running for this one so you must have at least 2 years experience as a support coordinator.
You will be working from home, autonomously and this role would suite someone wanting to work a 4 day week.
To be successful in this role, you will need:
Minimum 2 years experience as a support coordinator or similar (LAC/Planner). Please only apply if you have the relevant experience.
Excellent communication skills, both written and verbal and the ability to communicate in a positive and collaborative way
Effective time management skills
Experience with keeping case notes and excellent report writing skills.
Ability to build strong working relationships and establish an ongoing rapport with clients and stakeholders
Professional and/or lived experience with disability
The ability to work autonomously and efficiently from home office
A mature, flexible and committed attitude to your clients and work
Results driven, solutions focused
You will be system savvy and comfortable with following policies and procedures
Reside in the Eastern suburbs of Melbourne or willing to travel to see clients
Preferred:
A relevant qualification in Community/Social Services, Allied Health, Mental Health, aged care highly regarded
Demonstrated knowledge and experience of the NDIS
