We are a growing NDIS-registered provider expanding our Support Coordination services. Our focus is on delivering high-quality, participant-centred support while building strong relationships with stakeholders across the sector.
The Role
We are seeking an experienced and motivated *NDIS Support Coordinator* to join our team. This role offers flexibility, autonomy, and the opportunity to grow your own caseload with the support of an established provider.
Key Responsibilities
* Coordinate supports in line with participant NDIS plans
* Assist participants to build capacity and achieve their goals
* Liaise with providers, families, and stakeholders
* Monitor budgets and service delivery
* Maintain accurate case notes and reports
* Support participants through plan reviews

About You
* Experience in NDIS Support Coordination (Level 2 or 3 preferred)
* Strong understanding of NDIS legislation and processes
* Excellent communication and organizational skills
* Ability to work independently and manage a caseload
* Commitment to participant-centred practice
Requirements
* Relevant qualifications (e.g. Community Services, Social Work, Disability)
* NDIS Worker Screening Check
* Working With Children Check (if applicable)
* Professional Indemnity & Public Liability Insurance (for contractors)
* Valid Driver’s Licence and access to a vehicle
Highly Regarded
* Existing participant caseload or referral networks
* Experience with complex participants (psychosocial, SIL, high needs)
