**Job Description**
As a Hospice RN, you will be responsible for providing direct patient care services to hospice patients and supportive care to their families. You will work closely with an interdisciplinary team to develop and implement individualized plans of care that meet the unique needs of each patient.
You will also be responsible for:
Reporting directly to the Administrator, Executive Director or Patient Care Manager
Serving as primary liaison between physician, patient/family and hospice team
Providing education to patient's caregivers on care for their loved one
Maintaining all assigned patient clinical records
Participating in company quality assessment and performance improvement programs
**Required Skills and Qualifications**
To be successful in this role, you will need to have:
A current RN license in the state of requested employment (or eligibility to obtain)
1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology
Demonstrated flexibility and composure while supporting others in a time of crisis
Ability to work independently and as part of a team
Excellent communication and interpersonal skills
