You’ll be working closely with our dedicated team of NPs, RNs, GPs, specialists, and allied health professionals to ensure no patient is left behind — whether they live at home, in aged care, or face barriers to accessing traditional healthcare.
What You’ll Do:
Provide expert, autonomous care across a variety of settings — from clinic to home visits to aged care facilities.
Collaborate with a progressive, supportive team committed to best practice and integrated care.
Lead and contribute to multidisciplinary care planning with a focus on dignity, continuity, and outcomes.
Build long-term relationships with patients, families and residential partners across the community.
Utilise technology-enabled platforms to deliver care flexibly and efficiently.
