Position Summary

The Nurse Practitioner (NP) will play an integral role in the care of the patients of Jane Finch Family Health Team (FHT). He/she will assist the FHT in achieving its vision, mission, values and goals. The NP will support patient care and the delivery of FHT programs working collaboratively within an inter-professional healthcare team. The NP will practice within his/her full scope of practice as outlined in the Standards of Practice by the College of Nurses of Ontario.

Roles and Responsibilities

Diagnosis and Treatment:

Work collaboratively with other team members to manage the needs of the patient roster of Jane-Finch FHT including same day appointments for acute illness.
Conduct individual patient assessment and treatment – health assessment, immunization, ordering laboratory-screening tests, diagnosis, treating illness and follow up, health promotion and disease prevention, counselling.
Conduct full patient assessment and prepare a plan for intervention and/or for referral to FHT programs as appropriate.
Follow-up on all ordered laboratory tests and diagnostic exams.
Share results of assessment and intervention with referring physicians and other Team clinicians as requested within a specified timeframe.
Arrange physician consultation for issues falling outside the NP’s scope of practice.
Recommend and arrange specialist referrals and appointments.
Provision of individual or family telephone advice to patients.
Provide patient instruction for specific diagnostic tests and therapies.
Implement various goal-oriented, time-limited intervention plans, including education in collaboration with patient and/or family and inter-disciplinary team.
Apply self-management and other strategies to support the development of an informed, activated patient.
Evaluate and document outcomes of individual patient care in collaboration with the team.
On a rotating basis, provide service at the after-hours clinic.
Provide acute same-day appointments using Advanced Access Appointment system.
Organize, implement and Conduct programs at the request of the Executive Director and/or Physician Lead.
Other responsibilities may be added from time to time at the discretion of the Executive Director and/or Physician Lead.
Organizational:

Participate as a clinical team member in the development and delivery of targeted programs including but not limited to chronic disease management, child and family health, case management, mental health and addictions, illness prevention and screening.
Provide timely and informative reports as directed and requested by the Executive Director and/or Physician Lead.
Observe, collect and record patient data in the EMR system.
Participate in team building, program development, and improvement activities as requested.
Participate in staff meetings, to exchange ideas, receive mutual support and monitor program outcomes.
When required, assist in the development and implementation of the clinical model and guidelines for collaborative care.
Comply with established personnel policies, procedures, standards and guidelines for operation.
Identify gaps and needed modifications in services.
Complete required reports and statistics quarterly and more frequently if necessary, for Ministry of Health and other stakeholders.
Education, Community Relations and Advocacy:

Facilitate referrals to other services as well as communication and networking between the primary care practice and hospital or home care.
Maintain relations with networks, related professional organizations and other related service providers.
Participate in building creative, supportive environments for FHT staff and for patients.
Represent the mission, vision, values and goals of the FHT in the community, including networks, meetings or presentations, as requested.
Assist in the training and supervision of new NP hires, students and/or volunteers as requested by the Executive Director and/or Lead Physician.
Help patients to identify and use health resources.
Involve patients in decisions about their own health.
Support patients in learning how to self manage care.
Initiate and support health education activities.
Identify community needs and resources.
Provide clinical leadership.
Training and Research:

Be willing to contribute to the training and clinical placement experience of health professional students or residents placed at the Family Health Team.
Be a willing participant in approved research studies.
Other:

Participate in established performance evaluation process and professional development goals.
Develop, update and maintain education resources.
Attend relevant conferences, workshops and other in-service events as approved.
Participate in community meetings.
Qualifications and Requirements:

A master’s degree in Nursing is required with a Primary Health Care Nurse Practitioner Certificate.
Current registration with the College of Nurses of Ontario as an RN(EC) required.
Current Membership with RNAO and evidence of liability insurance.
Up-to-date BLS/CPR training.
Experience and/or training in community health, chronic disease management, mental health and child and family health preferred.
Ability to work with patients from diverse cultural backgrounds.
Knowledge of client-centred philosophy, change management, basic principles and practices of community service delivery, short-term treatment options and chronic disease management models.
Knowledge of community resources connected to the provision of health promotion and illness prevention related services.
Demonstrated skills in assessment, program planning, implementation and evaluation.
Work effectively, both independently and as part of an inter-professional team.
Ability to adapt to change.
Excellent computer skills.
Ability to utilize an electronic documentation of health records using a Electronic medical Record system
Ability to work flexible hours (some evening work will be required).
Reporting Structure: The NP reports to the Executive Director for day-to-day operations. Professional guidance and support will be provided by the site physicians. The Lead physician oversees all major clinical/operational decisions regarding direction of FHT.

Location: This position is located at 1 Yorkgate Mall, Toronto, ON, M3N 3A1

Hours of Work: Monday - Friday 9-5pm, but there will be a requirement to work one after-hours clinic (5-8 pm) every other week.

Status: Permanent, Full-time - to fill an upcoming vacancy.
