Job Description
Diagnosing and treating patients with minor illnesses, ailments and injuries history taking, physical examination, problem-solving and clinical decision-making, to establish a diagnosis and management plan Prioritise and triage the needs of patients accordingly, making any necessary referrals for investigations Clinical care, including: observations, assessment and reviews (CHD, LD, COPD, asthma, hypertension, diabetes), cervical screening, immunisations, injections, bloods, foot checks, ECG, NHS Health checks, peak flow, pill check, stitches and clips removal, urinalysis and wound care Clinical supervision and training of students Working under National Protocol for delivery of Covid 19 and Flu vaccinations Competency assessment and sign off for students Health promotion ******* screening, smoking cessation, healthy diet, physical activity, weight management, ****** health and family planning Long term conditions screening, diagnosis, ongoing management, and referral to other agencies in line with NICE guidance and Quality and Outcome Framework indicators Quality assurance and service support Continuous service improvement, including support with generating protocols and standard operating procedures Promote evidence-based practice through the use of the latest research-based guidelines and the development of practice-based research Effective signposting to health and social care services Engaging communities in healthcare; identifying and tackling barriers to access Recognise opportunities to reduce inequalities and unwarranted variation Follow and support development of pathways and standard operating procedures (SOPs) Handle sensitive and confidential information Stock management (equipment and immunisations) Responsible for maintaining the cold chain and integrity of vaccinations Ensuring the privacy, dignity and safety of individuals is maintained at all times Use of clinical IT systems, and maintaining accurate patient records Recognise and report issues relating to safeguarding vulnerable children and adults
