The Community Paramedicine Program is a proven model of care where paramedics utilise their skill set outside an emergency medicine setting to collaboratively support people with chronic disease management and prevention, health promotion, and primary healthcare navigation in the community.
As part of the Community Services Team, you will provide assertive outreach within the community to support people to access health and wellbeing services that improve their health outcomes.
