The Role
To carry out activities to ensure Health Care Practitioners (HCPs) and Patients are provided with high quality, safe and efficient nursing services within the Marion Medical Centre as a Practice Nurse Team Leader.
Key Responsibilities
Direct Patient Care Activities
In the Treatment Room (or otherwise) and in accordance with company Standard Operating Procedures (SOPs) and/or Standing Orders, conduct a range of direct patient care activities.
Direct patient care activities may include; administering vaccinations and assisting with catch up schedules, administering medications, conducting ECG, spirometry and other diagnostic tests, wound care, bandaging and plastering, removal of sutures, triage and emergency management, pre-employment medical screening and workcover injury management and set up and assist with HCP-led procedures.
Chronic Disease Management
Assist the GP with conducting health assessments, GP management plans, team care arrangements, reviews and initiate other chronic disease management tasks (e.g domiciliary medication management reviews) in accordance with Medicare requirements.
Provide the framework and support system to coordinate the care for patients with chronic condition(s).
Ensure collaboration with clinical and non-clinical support teams to provide a holistic health care approach.
