Primary Care Outreach Clinic - Primary Care Nurse (Nurse IV)
QUALIFICATIONS
Baccalaureate Degree in Nursing
Eligible for active practicing licensure as a Registered Nurse with applicable regulatory body in the province of Manitoba
Authorized prescriber notation (RN AP), ****** and Reproductive Health or willingness to obtain.
Five (5) years working as a nurse with three (3) years experience in Community Health, Primary Care, Emergency Department or a combination of each.
Experience in caring for persons with Substance Use Disorders
Experience in Motivational Interviewing and Health Behavior Change
Experience with electronic medical records
Experience in history of Clinical Decision Tool development and or use
Current Basic Cardiac Life Support (BCLS) Training as delineated in Prairie Mountain Health Policy PPG-00002 CPR Training
Province of Manitoba Class 5 Drivers Licence, or equivalent from province of residence, and access to a personal vehicle to provide service within Prairie Mountain Health
Demonstrated clinical skill in comprehensive history taking and physical assessments

POSITION SUMMARY:
Working in a Primary Care Provider-Nurse “teamlet” model the nurse provides, coordinates and supports primary care interventions that protect, maintain and improve the health and wellbeing of persons, including those individuals with demonstrated difficulty meeting their primary care needs within their present social circumstances.
The nurse, in case management of clients, leads in collaborative care planning and primary care service coordination through working cooperatively with client’s formal and informal support systems.
Using a harm reduction approach the clinic emphasizes health promotion, chronic disease self-management and self-advocacy for community resource mobilization. Primary Care services include:
substance use/addictions medicine; screening and treatment of Sexually Transmitted and Bloodborne Infection (STBBI) including Hepatitis C and HIV; wound care; reproductive health and prenatal care.
The Primary Care Outreach Clinic Nurse as the case manager and team lead is expected to align their practice understanding the principles of nursing leadership and applying them towards balancing the provision of direct care with engaging partners (internal and external) in the best interest of the client’s overall health status.
RESPONSIBILITIES:
Services to Client/Staff/Community
Provide priority primary care to individuals, groups and/or community.
Perform comprehensive health histories and assessments.
Work autonomously in nurse managed clinical care.
Work collaboratively to deliver team-based care requiring Acts outside of RN Scope of Practice.
Use principles of population health in multidisciplinary collaboration to implement strategies that prevent/reduce illness, injury and preventable deaths, e.g. immunization, health promotion and harm reduction interventions.
Partner and collaborate with internal and external partners that support the enhancement of client psychosocial function while maintaining primary care delivery role and function.
When required, provides case management of complex clients
Promote care access to higher risk clients (e.g. elderly, adolescent, mental health diagnosis) and groups experiencing lower health status due to correlated social health determinants (e.g. lower income, education, language barrier, housing instability).
Offer clinical services and health promotion in nontraditional settings using mechanisms and technology that facilitates access to care.
Apply culturally-relevant and appropriate approaches with people from diverse cultural, socioeconomic and educational backgrounds, ****** minorities, and persons of all ages, genders, health status and abilities.
Fosters independence and accountability for self-management of chronic conditions according to the client’s capacity and abilities.
Document and communicate accurate and pertinent client information in a timely manner.
Maintain effective therapeutic relationships and boundaries with clients.
Participate in the training and evaluation of volunteers and informal supports.
