The Opportunity

POSITION: Project Manager –ALC Transitions- Social Prescribing

CLASSIFICATION: Full-Time (37.5 hours per week), Fixed Term Ending March 31, 2027

ESCC is seeking a Project Manager – ALC Transitions-Social Prescribing Comprehensive Wrap-Around Approach to Transition Seniors from Alternate Level of Care (ALC) Spaces to Community to provide strategic and operational leadership for a community-based, wrap-around model supporting seniors transitioning from Alternative Level of Care (ALC) settings back into the community.

This role is ideal for a systems-oriented project leader who thrives in complex, cross-sector environments and is motivated by improving continuity of care for older adults.

Key

Project Leadership & Implementation

Provide overall coordination and oversight of the ALC transition model, ensuring activities align with project objectives, timelines, and deliverables.
Oversee and manage service partner contracts related to Seniors Mental Health Services, In-Home Supports, Transportation, Food Security, and Program Evaluation, ensuring compliance with agreements, performance expectations, and program outcomes.
Co-develops and aligns comprehensive wrap-around non-medical supports to transition seniors from Alternate Level of Care (ALC) spaces to community referral pathways, alignment with existing programs, including Social Prescribing, to ensure coordinated, integrated, and client-centered services
Develop and formalize partnerships, including the creation and management of Memorandums of Understanding (MOUs) with relevant partners.
Apply systems-thinking to identify interdependencies across health, social, and community-based sectors and proactively address implementation barriers.
Ensure alignment and accountability based on the Assisted Living Alberta (ALA) grant agreement.
Partnership Development & System Integration

In partnerships with the program team, identify, navigate, and address systemic barriers that impact client access, care coordination, and service delivery.
Cultivate and strengthen strategic relationships with partners across acute care, primary care, home care, mental health and addictions, and community-based organizations to build shared knowledge, support collaboration, and mobilize coordinated approaches for project development, partnerships, and referral pathways.
Support the development of shared processes, referral pathways, and communication protocols to reduce duplication and fragmentation.
Collaborate with system and community partners to strengthen continuity of care across transitions from hospital to community, serving as the primary liaison to support model development, project operations, system navigation, and evaluation activities.
Performance Management, Monitoring & Reporting

Oversee project performance monitoring to track referrals, outputs, outcomes, and key risks associated with supporting older adults with ALC designation to transition safely from hospital to community.
Collaborate with partners to support project data collection, documentation, and reporting in alignment with grant agreement requirements, including tracking client referrals, activities, and outcomes related to non-medical coordination, navigation, and community-based supports.
Support evaluation and continuous learning by documenting identified trends, lessons learned, and opportunities for improvement.
Contribute to performance measurement efforts in collaboration with Healthy Aging Alberta and other evaluation partners, as applicable.
Financial Oversight & Risk Management

Monitor project budgets and expenditures to ensure funds are used in accordance with the grant agreement and support the delivery of non-medical coordination, navigation, and community-based supports that enable timely and safe hospital discharge for older adults with ALC designation.
Work with partners to identify, assess, and manage project and system risks that may impact service delivery, partner coordination, referral pathways, or the ability to support successful transitions from hospital to community, escalating issues and supporting mitigation strategies as needed.
Support financial and narrative reporting to funders, including performance updates and risk reporting.
Communications & Knowledge Mobilization

Support internal and external communications related to the project, including partner updates, reporting, and information sharing with health, community, and government stakeholders involved in supporting older adults transitioning from hospital to community.
Contribute to knowledge-sharing efforts that highlight learnings, outcomes, and opportunities for scale or replication.
Promote awareness and understanding of the coordinated community-based wrap-around model among health system, community, and government partners to strengthen engagement, collaboration, and appropriate referrals for older adults with ALC designation transitioning from hospital to community
Organizational Supports

Actively engage in ESCC team building activities and/or initiatives that support the growth and development of ESCC.
Represent ESCC at community events as needed.
Other activities as assigned.
Education and Experience

Post-secondary degree in health administration, social sciences, public administration, or a related field.
A minimum of three (3) years of experience in a similar role.
An equivalent combination of education and community experience will be considered.
Experience working with cross-sector partners, including health systems and community-based organizations.
Skills and Knowledge

Proficient with Office 365 and relevant applications including Outlook, Teams and Sharepoint.
Extensive experience with Microsoft Office Suite utilizing Word, Excel, PowerPoint and other applications.
Strong project management skills, including planning, budgeting, coordination, monitoring, and reporting.
Demonstrated ability to work within complex, multi-stakeholder environments.
Knowledge of seniors’ services, health systems, or community-based social supports is an asset.
