Job Description
In this position, you will work closely with patient's physicians and other medical staff to determine treatment, set patient goals, and discuss treatment plans. You will analyze the best medications or therapy to help patients reach their goals and prescribe medications while monitoring patient intake.
You will document patient records and progress, provide patient counseling, educate patient, patient's family, significant others, and relevant parties about patient progress, treatment, and future goals. A strong understanding of population health models of care is essential for this role.
