Registered Nurse
About the Role:
This position requires a skilled and compassionate Registered Nurse to conduct comprehensive health assessments for patients in their homes, develop individualized care plans, and provide education and support to patients and their families.
Key Responsibilities:
Conduct thorough health assessments for patients in their homes
Develop personalized care plans based on patient needs, preferences, and clinical guidelines
Monitor and update patient progress at regular intervals, making adjustments to care plans as needed
Provide expert guidance and support to patients and their families regarding managing chronic conditions, medication management, and other health concerns
Collaborate with healthcare providers for referrals and optimize patient care
Maintain accurate and detailed records of patient conditions, treatments, and outcomes in the Service Plan (ISP)
Respond to patient inquiries and provide advice on home care best practices
Promote patient access to community resources and services as required
Ensure compliance with all relevant regulations and organizational policies related to nursing care
Qualifications:
RN with a current NZ Annual Practising Certificate
Minimum 5 years of nursing experience, preferably in home care, geriatrics, or community health
Ability to work independently and manage patient care in a home setting
Strong communication and interpersonal skills for interacting with patients, families, and healthcare teams
IT literacy and excellent organizational skills with attention to detail
Able to maintain a compassionate and empathetic approach in challenging situations
Current & clean driver's license and reliable transportation for patient visits
Preferred Qualifications:
Experience in wound care, palliative care, or chronic disease management
Physical Requirements:
Ability to travel to patients' homes within a designated service area
