Join Our Home Health Team – RN On-Call (24/7) – Hi Tech RN Visit & Hi Tech Skilled Nursing
Join our dedicated home health team as a Registered Nurse (RN) On-Call Nurse (24/7), providing Hi Tech RN Visits and Hi Tech Skilled Nursing, where your expertise will make a meaningful difference in patients’ lives across diverse care settings. This role allows you to provide exceptional nursing care in a flexible, on-call capacity, ensuring patients receive support around the clock.
Key Responsibilities
Respond promptly to on-call nursing needs, providing assessments and interventions for patients in their homes or care facilities through Hi Tech RN Visits and Hi Tech Skilled Nursing.
Conduct comprehensive patient evaluations, including vital signs, physical examinations, medication review, and care planning.
Manage complex cases including IV therapy, catheter care, wound care, and pain management.
Collaborate with physicians, case managers, therapists, and family members to develop and update individualized care plans.
Monitor patient conditions and identify changes requiring medical intervention.
Document patient interactions accurately in EHR/EMR systems, maintaining HIPAA compliance.
Provide patient and caregiver education on medication management, chronic disease management, dementia/Alzheimer’s care, and safety precautions.
Perform triage assessments and coordinate emergency or outpatient services as needed.
Qualifications
Valid RN license with current state licensure.
Experience in home health, Hi Tech Skilled Nursing, medical-surgical, or acute care nursing preferred.
Strong clinical assessment and critical thinking skills.
Familiarity with EMR/EHR systems for documentation.
Knowledge of ICD-10 coding preferred.
Experience with elderly patients, individuals with disabilities, or dementia care is a plus.
BLS certification required; additional certifications such as IV therapy or wound care are advantageous.
