Pay Range

Commensurate with Experience.

The program works alongside Hospitals in Partnership to support continued care. These partnerships focus on providing care to clients for 90-120 days after they are discharged from the Hospital. This program is designed to ensure clients are taken care of at a pace that is personable, comfortable, and not rushed.

Nurses who work with this program conduct a comprehensive head to toe assessment, and work in collaboration with the hospital through clinical leads at UHN or Home Care Specialist from VHA to review the clients needs. This is a home to home role, therefore a valid driver's license and vehicle are required. #INVHANS #LI-VHA

Key Areas of Accountability

VHA's Registered Nurses and Registered Practical Nurses are required to do the following tasks, but not limited to:
• Assess, Monitor, and Makes Care Planning/Care coordination
• Evaluate care plan interventions through on-going reassessment and evaluation results.
• Ensure the quality of care of patients by identifying and acting on factors which affects practice autonomy and delivery of quality care.
• Demonstrate professionalism, leadership, judgment, and accountability in interactions with client and families.
• Support safe client care through the development of knowledge and skills in accordance with our principles of continuous quality improvement.
• Inform nursing supervisor of any client activities and relevant concerns or changes in the client or the client care plan and informs/submits appropriate documentation to the case coordinator.
• Maintain documentation standards according to the College of Nurses of Ontario.

Required Qualifications:
• Registered and in good standing with the College of Nurses of Ontario (CNO).
• Membership of your Professional Association preferred (RNAO/WeRPN).
• RN diploma/degree or RPN diploma from a recognized university or college.
• Hold a valid First Aid or CPR Certification.
• A moderate level of wound care or venipuncture knowledge and experience is an asset.
• Acute Care and Medical-Surgical nursing experience is an asset.
• Knowledge of community nursing is an asset.
• Demonstrate effective communication (written and verbal) and documentation skills.
• Demonstrate reliable attendance, punctuality, and availability for the role.
• Commitment to modeling lifelong learning and professional growth and can apply critical thinking skills to provide client care.
• A valid Ontario driver’s license (G License) and a reliable vehicle for traveling is required.
