San Diego County Health Care Navigator

Job Summary:

Provide transportation to patients from diverse social, cultural, and economic backgrounds.

Screen patients for eligibility for financial assistance programs.

Collect specified co-pays and service fees payments.

Maintain accurate records, including patient data and daily vehicle logs.

Requirements:

Valid California Class A Driver's License required.

Bilingual English/Spanish preferred.

Ability to work effectively with patients from diverse backgrounds.
