Position Summary

The Senior Clinical Care Coordinator is responsible for providing clinical leadership and coordinating high-quality, person-centred in-home care for older people funded under the Support at Home program. The role ensures care delivery aligns with reablement and wellness principles, individualised budgets, and the Aged Care Quality Standards, while supporting clients to live safely and independently at home.

This position provides clinical oversight for clients with complex needs, supports care planning and service coordination, and contributes to clinical governance, risk management, and continuous improvement across Support at Home services.

Key Responsibilities

Clinical Leadership & Care Planning

Provide senior clinical oversight for Support at Home clients, including those with complex or changing clinical needs
Undertake comprehensive clinical assessments, reassessments, and care plan reviews
Ensure care plans align with Support at Home classifications, individual budgets, and reablement-focused outcomes
Identify and manage clinical risks, deterioration, and escalation pathways
Support at Home Coordination

Coordinate services across nursing, personal care, allied health, and support services
Support flexible service delivery in line with Support at Home funding arrangements
Monitor service utilisation and clinical appropriateness within individualised budgets
Support transitions of care, including hospital discharge, step-up/step-down care, and end-of-life planning
Quality, Compliance & Clinical Governance

Ensure care delivery complies with the Aged Care Quality Standards, Support at Home program requirements, and organisational policies
Contribute to clinical governance activities, audits, and quality improvement initiatives
Maintain accurate, timely, and compliant clinical documentation
Support incident management, feedback, and continuous improvement processes
Client, Family & Stakeholder Engagement

Partner with older people and their families to support informed decision-making and goal-based care
Liaise with GPs, hospitals, allied health providers, and community services
Respond to clinical concerns and complaints with professionalism and empathy
Staff Support & Capability Building

Provide clinical guidance, mentoring, and supervision to care coordinators and frontline staff
Support workforce capability through education and best-practice guidance related to Support at Home
Contribute to onboarding and ongoing competency development
Essential Qualifications & Experience

Registered Nurse (current AHPRA registration) or relevant allied health qualification
Demonstrated experience in community or in-home aged care
Experience coordinating care for older people with complex health needs
Strong understanding of the Support at Home program, reablement principles, and consumer-directed care
Sound knowledge of the Aged Care Quality Standards and clinical governance
Skills & Attributes

Strong clinical assessment and care coordination skills
Excellent communication and relationship-building abilities
Ability to balance clinical priorities with funding and service considerations
Person-centred, strengths-based, and culturally responsive approach
High level of organisational and time-management skills
Desirable

Postgraduate qualification in aged care, community nursing, or gerontology
Experience in transition from Home Care Packages to Support at Home
Experience supporting clients with dementia, chronic disease, or palliative care needs
Other Requirements

Current driver’s licence and ability to travel to client homes
National Police Check and relevant clearances
