About the Role

We are seeking a registered health care professional (Registered Nurse, Occupational Therapist or Social Worker) to work in a senior coordinator role supporting tāngata hāpori/ tāngata whaiora accessing our service, Te Whakawhitinga.

Te Whakawhitinga works alongside people in Hamilton experiencing a significant housing issue and attempts to connect them with the right sustainable supports to meet the need - this might mean accessing supports from within our team of clinicians or accessing the right community resource.

The senior coordinator has proven clinical competence within Mental Health or similar services and works with tangata whaiora to provide whaanau-centred care. The role is primarily clinical but incorporates some clinical leadership and education components.

Key responsibilities of the roles include:

Conducting an initial assessment of needs in partnership with tāngata hāpori/ tāngata.
Working with tāngata hāpori/ tāngata whaiora, their whaanau and providers to produce plans together, to best meet their needs and preferences.
Coordinating the tāngata hāpori/ tāngata whaioras plan to direct, enable and evaluate a range of interventions, from a range of providers to meet the goals identified.
Developing and building strong interagency relationship across Iwi, Māori NGO providers, Government organisations, Non-Governmental Organisations and the Mental Health and Addictions Sector.

Te Whakawhitinga is a co-ordinated service aligned to the Health New Zealand | Te Whatu Ora Waiora vision and underpinned by Kotahitanga. It is part of the wider mental health and addiction services in Hamilton. The Kotahitanga values of unity, togetherness, shared vision, working together, trust, relationships, collaboration and integrations are fundamental to the service model of Te Whakawhitinga.

We intend to meet the needs of tangata hāpori/ tangata whaiora across the housing continuum, who are experiencing health, social and environmental issues that could put tenancies at risk or have resulted in homelessness.

We will do this by being a tangata hāpori/ tangata whaiora led service that;

helps a person identify their goals and aspirations that will achieve mauri ora
supports access to community resources and services that will meet these goals
works in close partnership with tangata hāpori/tangata whaiora and other agencies in a transparent and coordinated way.
The team includes coordinators, ******* and drug clinicians, an Occupational Therapist, community support workers and Takawaenga.

About you

Registration with the applicable registration board with a current annual practicing certificate.
A minimum of 5 years post graduate experience in a mental health, community health or primary care setting.
An understanding of clinical governance and evidence based practice.
Experience in professional supervision.
Experience in service improvement.
Competency with te ao Māori, tikanga, and te reo Māori or a commitment to starting your journey and taking ownership of your learning and growth.
Highly developed interpersonal communication and negotiation skills including demonstrated ability to build productive working relationships with colleagues to achieve required outcomes.
Ability to take a proactive role with other organisations to achieve sustainable outcomes for tāngata whaiora.
Current full drivers licence.
