SNSWLHD is looking for a Rural Generalist for Hospital in the Home (HiTH), with a Fellowship from ACRRM or RACGP, with training or experience relevant to the role.

Southern NSW Local Health District covers a large area of the state, from Goulburn and Crookwell to the Victorian border, and surrounds the ACT on three sides, a total of 44 537sq kms. It extends from the pristine beaches of the NSW South Coast, the farmlands of the Southern Tablelands, across the picturesque Great Dividing Range and the spectacular Snowy Mountains. We are home to four large Aboriginal nations – the Gundungurra, Ngunnawal, Ngarigo and Yuin.
Southern NSWLHD comprises of 2 Networks - Inlands centred around Goulburn, Monaro, Queanbeyan and Cooma, AND Coastal centered around Bega, Batemans Bay and Moruya in the far south-east corner of NSW and the Integrated Care Network.
Southern NSW Local Health District covers a large area of the Southern half of NSW. Surrounding the ACT on three sides and all the way down to the Victorian border, it extends from the pristine beaches of the NSW South Coast, the farmlands of the Southern Tablelands, across the picturesque Great Dividing Range and the spectacular Snowy Mountains. We have 18 facilities and our hospitals take care of more than 200,000 people. As we are a major local employer, our staff are a part of our local communities.
What you'll be doing
Provide contemporary comprehensive Physician services and Medical Governance for optimal patient outcomes for the newly established Hospital in the Home (HITH) Model of Care. The role will establish and maintain a district wide multidisciplinary approach to care and work in partnership with Nurse Practitioners and multidisciplinary team to deliver holistic integrated care in line with HITH PD2025_004.
You will be employed by Southern NSW Local Health District to provide the delivery of acute and sub-acute treatments by health care professionals in the patient’s home, residential aged care facility, community setting or the Hospital in the Home (HiTH) unit. The position is a district wide position servicing HITH patients across SNSWLHD. Secondary appointments within other facilities may occur by mutual agreement.
Essential Criteria:
Current Registration as a Medical Specialist with the Australian Health Practitioner Regulation Agency (AHPRA).
Selection Criteria:
Eligible for registration with the Medical Board of Australia as Emergency or Specialist General Physician and the ability to accept care of a variety of clinical conditions.
Demonstrated ability to work effectively and provide clinical leadership of a multidisciplinary team including Nurse Practitioners.
Demonstrated effective clinical communication through the use of information technology and written, verbal and interpersonal skills.
Demonstrated ability and willingness to partner in collaborative care models with relevant specialists.
Demonstrated commitment to teaching, continuing education, research and quality improvement.
Demonstrated understanding and commitment to all aspects of clinical governance.
Demonstrated ability to consistently display a commitment to, and compliance with, and leadership in high quality Customer Service, Workplace Diversity, Occupational Health & Safety and Industrial Democracy principles, practices and relevant legislation relating to these areas and an understanding of and commitment to the organisation's values.
Evidence of a current unrestricted drivers’ licence and the ability to maintain. Ability and willingness to drive/travel as the role requires. This may involve driving long distances and overnight stays.
