Job Description
The Support Coordinator is responsible for assisting NDIS participants to understand, implement, and optimise their NDIS plans. This role focuses on building participant capacity, connecting participants with appropriate supports, managing risks, and ensuring services are coordinated, effective, and aligned with participant goals.
This position is remunerated based on billable hours delivered, with clear expectations regarding productivity, documentation, and professional standards.
Key ResponsibilitiesParticipant Support & Coordination
Support participants to understand and implement their NDIS plans
Assist participants to identify goals and build capacity to manage supports independently
Source, link, and coordinate NDIS‑funded and community‑based supports
Facilitate service agreements between participants and providers
Monitor service delivery to ensure supports align with participant goals and plan budgets
Risk & Complex Needs Support
Identify and manage risks impacting participant safety, wellbeing, or plan sustainability
Support participants experiencing complex needs, service breakdowns, or crisis situations
Escalate concerns appropriately and work collaboratively with internal and external stakeholders
Stakeholder Engagement
Build strong working relationships with participants, families, carers, and providers
Liaise with NDIS planners, Local Area Coordinators (LACs), and other professionals as required
Advocate for participant choice, control, and rights
Documentation & Compliance
Maintain accurate, timely, and NDIS‑compliant case notes and records
Ensure all billable activity meets NDIS requirements and organisational standards
Track plan utilisation and support participants to use funding effectively
Contribute to reports, reviews, and plan reassessments where required
Billable Performance Expectations
Meet agreed billable‑hour targets aligned to the role and employment arrangement
Accurately record billable and non‑billable activities
Manage time efficiently to maximise participant outcomes and billable productivity
Qualifications & ExperienceEssential
Diploma or higher qualification in Community Services, Disability, Social Work, Human Services, Mental Health, or related field OR equivalent experience
Demonstrated experience in NDIS Support Coordination
Sound understanding of NDIS legislation, Practice Standards, pricing arrangements, and service pathways
Strong written documentation and communication skills
Desirable
Experience supporting participants with complex needs
Knowledge of local service networks and community supports
Experience working in a billable or target‑based service model
Compliance Requirements
NDIS Worker Screening Check
National Police Check
Valid driver’s licence and access to a reliable vehicle (if applicable)
Ability to work in accordance with NDIS Code of Conduct and Practice Standards
