As a Band 5 Staff Nurse in our Acute Frailty Assessment Unit (AFAU), you'll play a crucial role in providing high-quality, compassionate care to our elderly and frail patients.

AFAU is a Same Day Emergency Care (SDEC) service which is a 12 patient assessment space. We offer rapid assessment and care for older adults within a safe and dementia friendly environment. Patients will remain in the unit for no longer than 10 hours and thereafter discharged home with a plan of care or transferred to the relevant ward for further treatment.

You'll be part of a dynamic multidisciplinary team, using your clinical skills and knowledge to assess, plan, and deliver person-centred care. We are seeking a dedicated and compassionate Band 5 Staff Nurse to join our Acute Frailty Assessment Unit. In this role, you will be responsible for providing high-quality nursing care to elderly and frail patients, working as part of a multidisciplinary team to assess, plan, implement, and evaluate patient care.

Clinical Practice:
• Develop, implement, and evaluate personalised care plans for frail elderly patients
• Administer medications and treatments as prescribed, ensuring proper documentation
• Monitor patients' conditions, recognise deterioration, and respond appropriately
• Perform clinical procedures within your scope of practice
• Ensure compliance with infection control policies and maintain a safe environment for patients and staff.

Patient Care and Communication:
• Provide compassionate, person-centred care that promotes dignity, independence, and well-being
• Communicate effectively with patients, families, and carers, providing education and support about care and treatment
• Advocate for patients' needs and preferences within the multidisciplinary team
• Participate in family meetings as required

Teamwork and Collaboration:
• Work collaboratively with the multidisciplinary team, including doctors, therapists, social workers, and other healthcare professionals
• Contribute to effective handovers and ensure continuity of care
• Participate in team meetings and quality improvement initiatives
• Mentor and support junior staff, students, and newly qualified nurses

Documentation and Administration:
• Maintain accurate, comprehensive, and up-to-date patient records
• Contribute to discharge planning and ensure safe transitions of care
• Complete risk assessments and incident reports as required
• Participate in audits and data collection for service improvement

Professional Development:
• Maintain registration with the Nursing and Midwifery Council (NMC)
• Engage in continuous professional development, keeping up-to-date with current best practices in frailty care
• Participate in mandatory training and seek opportunities for specialised learning in geriatric care
• Contribute to the development of policies and procedures within the unit.

Quality and Safety:
• Adhere to all relevant NHS and Trust policies, procedures, and guidelines
• Participate in clinical governance activities and contribute to maintaining high standards of care
• Report concerns and incidents promptly, contributing to a culture of openness and learning
• Support the implementation of evidence-based practice and quality improvement initiatives.

Person Specification:
• Registered Nurse with current NMC registration
• Degree in Nursing or equivalent
• Knowledge of the specific needs of frail elderly patients
• Excellent communication and interpersonal skills
• Ability to work effectively in a multidisciplinary team
• Good organisational and time management skills
• Commitment to patient-centred care and promoting dignity and independence
• Willingness to engage in continuous professional development.

Desirable:
• Post-registration experience in elderly care, acute medicine, or a related field
• Additional qualifications or training in gerontology or frailty care
• Experience with electronic patient record systems
• Understanding of current issues and trends in frailty care and geriatric medicine.
