About the Opportunity

An exciting opportunity exists for a Staff Specialist in Geriatrics to come and work part-time (0.63 FTE) for 6 months with a team of motivated clinicians who have a shared vision aiming to prevent avoidable presentations, admissions and readmissions to hospital of people living in the community in either their own home or a RACF.

The Community Older Person’s Intervention and Liaison Team (COPILOT) provides urgent medical, nursing and allied health assessment and interventions for non-life-threatening conditions.

This position sits in the community and has strong support from a medical Head of Department, a Team leader as well as a multidisciplinary team of medical, nursing and allied health.

You will have experience in working with geriatric patients as well as good communication, relationship building and decision-making skills.

The role is autonomous however there are clear escalation processes in place and support systems within Primary and Community Health

Whilst the position is for 6 months there is the possibility of extension.

What you'll be doing

Provide clinical management and treatment of patients in the community or a SWSLHD facility under your care. This care will be provided within the clinical privileges granted by the CE on the advice of the SWSLHD Medical and Dental Appointments Advisory Committee.

Current medical practitioner, registered, or eligible for registration, with the Medical Board of Australia and Fellow of the Royal Australian College of Physicians in the specialty of Geriatrics.

Demonstrated current clinical practice of a high standard and ability to adapt clinical practice in accordance with contemporary evidence based best practice in acute community geriatric medicine.

Demonstrated commitment to quality improvement, patient safety and risk management.

Demonstrated commitment to continuing self-education and development, including evidence of participation in continuing medical education/ continuing professional development.
