About the Role


An opportunity exists to assist older people and their families to design and implement various levels of high-quality home care services in consultation with the Home Support Services team.



Responsibilities include but are not limited to:

Providing advice to consumers and their carers and or advocates, regarding the selection, self-management, and monitoring of services under a CDC model
Working with eligible consumers and their carers and or advocates, to identify goals, provide holistic assessment, care coordination and monitoring of services for older people using an individual, consumer focused approach.
Meet with consumers/families and or advocates to discuss future care needs if a Home Care Package is unable to provide care for the consumer long term and is unable to meet the consumers’ needs.
Ensuring effective relationships are developed and maintained with stakeholders, including but not limited to:
Consumers and families and or advocates
Service providers
Government agencies
Maintaining contact with consumers and/or families/ advocates to ensure that services and budgets are monitored and re- assessed as appropriate
Maintaining all service plans in the home with every service on the plan clearly describing roles and responsibilities of each person involved in the consumers care including carers and advocates.
The ideal candidate will have:

Certificate III in Aged Care, Community Services or equivalent (desirable not essential)
Current registration as a Registered Nurse/Enrolled Nurse/Allied Health (desirable not essential)
Demonstrated knowledge of relevant legislation, including funding and policy protocols, standards, and processes
A basic knowledge of legislation in aged care and the legal implications of actions taken, e.g. Elder Abuse, Code of Conduct for Unregistered Health Practitioners and duty of care.
Demonstrated experience in all aspects of case management including comprehensive assessment, re assessments, planning of services within budget, problem solving and the provision and evaluation of services for older people
Highly developed interpersonal, communication and negotiation skills to maintain positive working relationships with stakeholders, handle sensitive matters with tact and diplomacy and maintain a high level of confidentiality.
A current drivers' licence.
You must also be willing to obtain a National Police Certificate before commencing employment as well as a functional assessment.
