As a Support Coordinator with In2Change, you will work alongside participants to navigate the NDIS and build the skills and connections needed to live an ordinary life. You will play a key role in supporting participants to build their capacity to engage with services, strengthen informal networks, and increase their independence and choice and control.
Your responsibilities will include:
-Assisting participants to understand and implement their NDIS plans
Connecting participants with informal, community, mainstream, and funded supports
Building participant capacity to exercise choice and control to make informed choices and increase independence and autonomy
Build and maintain strong relationships with participants, nominees and providers and community networks
Supporting positive outcomes through a collaborative, person-centred approach
We are looking for someone who aligns strongly with our values and approach:
Demonstrated commitment to the social model of disability
Strong belief in the principles of an ordinary life for all people
Experience in Support Coordination or a related role within the NDIS or community sector
Excellent communication, organisation, and relationship-building skills
Ability to work both independently and collaboratively within a dynamic team
Excellent time management and organisational skills and the ability to manage a complex caseload and competing priorities is essential
