About the role
Assist NDIS participants in coordinating and implementing support services based on their NDIS plan, using a person-centred approach. Build strong relationships with participants, their families, NDIS coordinators, planners, and service providers to ensure effective support and service delivery. This is a permanent part time (three days per week – any days within Monday to Friday) position that is located from Hobart. You will have the flexibility to work from home or in our Hobart office, however, will be required to meet with clients across the Hobart region as required.
Key responsibilities
Strengthen and enhance our client's ability to coordinate supports and participate in the community
Regularly monitor our client's NDIS plan to ensure services are being delivered within their allocated budget
Ensure mainstream services meet their service obligations to clients (i.e. housing, education, justice, health)
Provide the client and the NDIA with reports on outcomes and success indicators, within the agreed reporting frequency
Skills & experience
Experience as an NDIS Support Coordinator or similar
Unrestricted drivers licence and willingness to travel as required
Excellent verbal communication skills
Strong written and computer skills for the creation of plans, programs reports, correspondence and other documents
Established time management, problem solving and conflict management skills
Ability to think creatively and use initiative
Successful candidates will be required to clear probity checks including National Criminal History Record Check, NDIS Workers Screening Check and a TAS Working with Vulnerable People Check.
