Would you like to utilise your clinical knowledge, health-related degree or disability or healthcare sector experience in a rewarding client-centred role?
We have opportunities to work as a Support Facilitator, where you will champion the needs of our clients and drive positive outcomes. In this role, you will provide professional support, conduct needs assessments, reassessments and arrange support packages for clients. Working together with clients and whanau, your aim is to ensure that the right outcomes are being achieved, in line with individual goals and needs.
We are Kaikaranga (formerly Taikura Trust). Since 2002, we've been supporting people with disabilities through the allocation of services and information to remove barriers, promote independence and to help them live their best life.
About the role …
After conducting indepth needs assessments, you will identify and prioritise client support needs, and coordinate a tailored support package.
Supporting people with disabilities, that result in a wide range of needs and circumstances.
Collaborate with a range of stakeholders, team members and providers, to ensure the needs and goals of our clients are understood and achievable.
Maintain high quality documentation and record relevant information in line with practice and processes.

About you …
Strong verbal and written communication skills, with the ability to relate to a range of people from diverse cultural backgrounds.
You have proven experience in coordinating natural and formal supports and services for people with a range of physical, intellectual and sensory needs
Commercially savvy and capable of navigating operational IT systems
Passionate about improving the lives of those living with disabilities and commitment to Kaikaranga's values.
You will need some experience working in the health / allied health / education or disability sector, or a relevant health-related qualification.
