Chronic Care Manager Nurse - Telehealth

As a Chronic Care Manager, either as a Registered Nurse or Licensed Practical Nurse, you will deliver the compassionate and personalized care that you are known for, all while enjoying the flexibility of a home-based position. Work Schedule Options
• Monday to Friday, 8:00 AM - 4:30 PM EST or 11:00 AM - 7:30 PM EST
• After six months, select from various schedules that suit your lifestyle, including 4 days x 8 hours, 4 days x + hours, or 4 days x 10 hours.
Your role as a Chronic Care Manager is pivotal in fostering enduring patient relationships. By engaging with the same patients monthly, you will offer consistent, personalized support that enhances their healthcare experience and empowers them to prioritize their well-being.
Key Responsibilities
• Conduct regular patient check-ins to discuss health changes, follow-up on appointments, and facilitate understanding of new medical instructions.
• Build trust and rapport with patients and their families, ensuring a supportive care environment.
• Serve as a key connector within the patient care team by communicating effectively with other healthcare providers and office staff.
• Utilize your nursing expertise to educate patients about their health priorities and conditions.
• Advocate for patients by addressing medical concerns and providing timely responses.
• Leverage technology to coordinate care, set goals, and manage resources effectively.
• Employ Electronic Medical Record (EMR) systems to document patient interactions and review prior office visits.
Required Skills
• Heartfelt compassion in patient care.
• Active RN/LPN license in any U.S. state (licensing fees covered for out-of-state care).
• A minimum of 4 years of nursing experience with chronically ill patients.
• Experience in home health or primary care settings is advantageous.
• Proficient in Electronic Medical Records (EMR) systems.
• Technologically adept and adaptable to new tools.
• Strong time management and communication skills.
Qualifications
• Compassionate and patient-centered approach to care.
• Licensed as an RN/LPN.
• Experience in nursing care specifically for chronically ill patients, with at least 4 years of relevant experience.
