Responsibilities
• Responsible for starting the patient's care, including nursing assessment, medication reconciliation, safety assessments and implementing standardized protocols.
• Administers prescribed medications (typically intravenously), obtains blood samples, inserts foley catheters and provides therapeutic interventions.
• Educates patient on how to manage their illness, prescribed medications, nursing interventions, and care plan.
• Identifies and acts on changes in patient's condition.
• Evaluates and revises care plans/assessments based on changes in patients and/or their environments.
• Documents all assessments, interventions and care coordination details.
• Participates in regular, in-service or special case orientations as needed.
• Participates in on-call rotation.
• Performs all other duties as assigned.

Qualifications
• Associate of Science in nursing.
• Current RN license, specific to the state(s) you are assigned to work.
• One year of adult acute inpatient medical, or medical/surgical care experience within the last five years.
• Current CPR certification.
• Valid driver's license.
• Reliable transportation and company required liability insurance.

Preferred
• Bachelor of Science in nursing.
• Experience providing RN services in the home, community or clinic.
• Adult medical critical care.

Financial Coordinator

Beth Abraham Center for Rehabilitation and Healthcare is actively seeking a full-time Financial Coordinator to work in the business office of our facility. The ideal candidate will be detail-oriented and be able to effectively multi-task; should also be computer savvy, have a good personality, and be able to work well with others. Familiarity with skilled nursing facility finances, as well as experience dealing with Medicaid, is preferred.

Must have healthcare office experience

The Financial Coordinator will be an integral member of our finance team, whose responsibilities will include:
• Meeting with residents/families to discuss their financial obligations
• Work with residents and families to ensure they are paid in full
• Make sure all admission agreements are properly signed
• Make sure all monthly NAMI (income contributions) are paid in full
• Other duties as required

REQUIREMENTS:
• Prior experience in a Nursing Home finance/Medicaid office
• Must have or be in the process of receiving a Bachelor's degree
• High comfort level to meet with residents/families to discuss and advise on their financial obligations
• Must be computer literate, and proficient in Microsoft Office
• Should be highly detailed and organized

Job Summary
In a Transfer Center role the Specialist will be the single point of contact for facilities/providers requesting transfer to a Monument Health hospital. The Specialist obtains patient information critical to determining the appropriate Monument Health provider and level of care while coordinating phone call between the referring and receiving providers. In the patient placement role, the Transfer Center Specialist will be responsible for facilitating patient flow within Monument Health, balancing bed requests with priority of need. This position works closely with referring hospitals, receiving hospital, and internal Monument Health physicians and caregivers. • Facilitates communications with referring and receiving providers.

• Acts as a customer service representative taking appropriate steps to meet the needs of all callers.
