About the Role
The Transitional Care Program (TCP) Coordinator plays a key role in delivering Lutheran Homes Group’s Transitional Care Program to a high standard, supporting older people as they move from hospital to residential or community-based care. Working with autonomy and in close partnership with SA Health, clinical teams, allied health professionals, families and external providers, this role is responsible for assessment, care planning, case management and service coordination to help consumers regain independence, improve wellbeing and transition safely to longer-term supports. It is a hands-on, relationship-focused role suited to a clinician who combines strong care coordination capability with sound judgement, professionalism and a commitment to person-centred, restorative care.

Pay Range- $45.60 per hour
Tax-free benefit: Up to $18,549 of your income tax-free through salary packaging

Key responsibilities include:

Coordinate the delivery of the Transitional Care Program in line with best practice, program guidelines and Lutheran Homes Group standards.
Provide assessment, support, care planning and case management for consumers and their support persons across residential and community/home-based TCP services.
Facilitate smooth transitions between hospital, residential care and community settings through effective discharge planning and coordinated service pathways.
Develop, implement and review restorative, person-centred care plans in collaboration with consumers, families, allied health professionals and the broader multidisciplinary team.
Act as the central point of contact for all stakeholders involved in each consumer’s care, including GPs, nursing staff, allied health teams and external service providers.
Coordinate referrals, streamline service access and advocate for consumers to ensure appropriate supports are in place.
Monitor consumer progress and adjust care plans and service delivery arrangements as needs change.
Maintain accurate records, reporting and administrative requirements, including program and financial reporting relevant to the TCP agreement.
Build and maintain collaborative relationships with internal teams, hospitals, community providers and other agencies to strengthen service integration and consumer outcomes.
Contribute to quality improvement, service evaluation and continuous improvement activities to enhance program effectiveness and consistency across LHG.
Promote the TCP program to relevant stakeholders and the community, supporting awareness and engagement.
Support a culture of professionalism, teamwork, customer service, cultural safety and consumer involvement in all aspects of care.
About You
Current registration, or eligibility for registration, as a Registered Health Practitioner with AHPRA and a current practising certificate, or equivalent professional eligibility relevant to the role.
At least 1 years’ post-registration experience in a relevant clinical setting.
Demonstrated experience in assessment, care planning, case management and care coordination.
Strong understanding of restorative, consumer-centred care and discharge planning principles.
Ability to work independently, prioritise competing demands and manage workloads effectively.
Excellent communication, stakeholder engagement, negotiation and problem-solving skills.
Experience working collaboratively within multidisciplinary teams and across service providers.
Strong documentation, reporting and administrative capability, with attention to detail.
Knowledge of quality improvement practices and a commitment to continuous improvement.
Sound understanding of relevant legislation, professional standards, codes and aged care quality requirements.
Commitment to cultural safety, respectful service delivery and meaningful consumer involvement in care.
Current driver’s licence and willingness to travel across sites; able to meet NDIS risk-assessed screening requirements (NDIS Worker Clearance or compliant NPC).
