We are seeking a Registered or Enrolled Nurse to join our dynamic Outpatients team. As a key member of our Planned Services team, you will play a vital role in providing exceptional patient care and communication skills. Your primary responsibilities will include working collaboratively with the healthcare team to achieve optimal patient outcomes, utilizing case management concepts to ensure seamless patient transitions, and providing strong communication and teamwork skills.
