Behavioral Health Care Coordinator
Join our dynamic team as a Behavioral Health Care Coordinator in the Care Management Department!
Responsibilities:
Provide behavioral health care coordination activities for Alliance members.
Establish and maintain effective working relationships with provider offices, County departments, and other community agencies, and internal stakeholders related to care coordination for members.

Our mission is to empower members to achieve their health and wellness goals through collaboration and facilitating member relationships with community providers. We bring the voice of the member into the room when collaborating with our fellow departments at the Alliance.
The Ideal Candidate:
We are looking to hire exceptional individuals who thrive in fast-paced environments, are well-organized and detailed, adapt well to changing situations and policies, and enjoy working with, empowering, and educating others. Bilingual candidates in English/Spanish or English/Hmong are highly desired for these positions.
Requirements:
Knowledge of health care systems, care coordination, customer service, and diverse needs of the Medi-Cal population.
Ability to understand the workings of the Alliance, communicate program mission, vision, and roles, learn Microsoft Excel and Alliance computer systems, interpret, explain, and apply policies, procedures, and guidelines, and document, summarize, and resolve members' concerns and inquiries.
Education and Experience: High school diploma or equivalent, three years of experience in care coordination in a health care setting, or an equivalent combination of education and experience may be qualifying.
