💰 $5,040 - $8,064 (Est.) 📍 Perth 🕐 6 days ago

Job Description

Flexible working environment with great salary sacrifice benefits
Work by the beach in world-class surroundings with onsite parking
Work for an organisation that is putting the focus back into Community
This is a coordination and care management role – not a direct support worker position.

Are you an experienced Care Coordinator or Client Manager who thrives in a fast-paced, evolving aged care environment? Do you believe every older Australian deserves to live well in their own home?

At Curtin Heritage Living, we're not just delivering care — as part of the new era of in-home aged care, we're building services that combine clinical expertise with genuine compassion.

About Us
Curtin Heritage Living has been transforming lives in aged care across home care, independent living, and residential care for over 45 years. As a trusted not-for-profit organisation, we're at the forefront of the new Support at Home program, creating innovative pathways for independence, wellness, and meaningful living.

The Role
As our Care Partner – Support at Home, you'll be the architect of personalised care journeys for our valued clients. This is a strategic coordination role requiring strong industry knowledge, sound judgement, and the ability to adapt as the aged care landscape continues to evolve. You'll:

Be the primary advocate and point of contact for clients across Support at Home, Fee for Service, and Brokerage programs
Design and manage comprehensive, individualised care plans aligned with the Aged Care Act 2024 and Strengthened Aged Care Quality Standards
Collaborate closely with our Clinical and Allied Health teams to deliver exceptional wellness and reablement outcomes
Strategically manage quarterly budgets, maximising utilisation through proactive planning and innovative service delivery
Champion client choice, independence, dignity, and rights in every interaction
Navigate ongoing sector reform with confidence, contributing to continuous improvement across the team
What You'll Bring
This role requires someone who brings both experience and adaptability. You'll need:
Demonstrated coordination experience in aged care, preferably in-home or community services — this is essential, not negotiable
Resilience and comfort with change — you've worked through sector reforms, shifting funding models, or organisational change and you've thrived, not just survived
Certificate IV in Aged Care, Community Services, or Individual Support, and/or a relevant tertiary qualification (a clinical background is highly regarded)
Strong understanding of aged care funding frameworks, particularly Home Care Packages and the Support at Home program
Excellent communication and relationship-building skills with clients, families, and multidisciplinary teams
Sound judgement, initiative, and the ability to manage competing priorities
A genuine commitment to enhancing the lives of older Australians
Current National Police Clearance (within three years)
Australian citizenship or valid visa with full working rights
Why Curtin Heritage Living?
Be part of a mission that truly matters — supporting older people in your local community to lead meaningful, independent lives
Join a values-driven team built on Client Focus, Community Attitude, Accountability, and Simplicity
Ongoing professional development and genuine career pathways within our expanding organisation
Work somewhere your expertise is respected, your voice is heard, and your growth is invested in
Help shape the future of in-home aged care as the sector undergoes its most significant transformation in decades
Ready to Apply?
If you're an experienced coordinator who's passionate about aged care and ready to bring your skills to a team that's leading through change, we'd love to hear from you.
Submit your resume and a cover letter that clearly outlines your relevant coordination experience and why you're the right fit for this role. Applications without a tailored cover letter addressing suitability will not be progressed.
