Harmony Healthcare is looking for a Clinical Case Manager to join our growing team. If you’re committed to improving the lives of individuals with complex care needs in their own homes, this role could be for you!

About The Role

As a Clinical Case Manager, you will play a vital role in ensuring our clients receive exceptional care tailored to their individual needs. You will be responsible for:

✅ Overseeing the clinical care of clients and ensuring high standards of service delivery

✅ Working closely with commissioners to take on new packages and support business growth

✅ Identifying and supporting staff to ensure continuity and quality of care

✅ Safeguarding clients, their families, and staff while maintaining compliance with CQC regulations

✅ Implementing and maintaining agreed care standards, reporting to the Head of Clinical Excellence

Key Responsibilities

Assessing the physical and mental health needs of clients and creating tailored care plans

Completing risk assessments and ensuring a safe care environment

Acting as a key point of contact for external agencies, office teams, clients, and their families

Conducting reviews to adapt care packages based on evolving client needs

Providing training, supervision, and appraisals for healthcare staff

Supporting recruitment, interviewing, and onboarding of healthcare professionals

Responding to concerns and complaints in a professional and timely manner

Participating in an on-call rota for out-of-hours support

What We’re Looking For

✔️ A Registered Nurse with a valid NMC PIN

✔️ A minimum of 5 years’ experience in health and social care, including community-based complex care

✔️ Strong leadership, team management, and problem-solving skills

✔️ Knowledge of CQC regulations and best practices in care provision

✔️ Excellent communication, organisational, and IT skills

✔️ A full UK driving licence and access to a vehicle
