We are a dynamic community-based health centre promoting social justice and health equity in the Regent Park neighbourhood and Downtown East area of Toronto. We operate out of several locations and provide a wide variety of services including: primary health care, chiropody, physiotherapy, health promotion, social work, community health, immigrant and refugee services, harm reduction programs, addiction and recovery services, Pathways to Education, infant and child development programs, parenting programs, and youth employment services. Background

The Inter-professional Primary Care Team (IPCT) brings together Regent Park CHC, St. Michael’s FHT, Inner City FHT, and Sherbourne Health FHT to enhance access to interdisciplinary primary care for individuals in the downtown east of Toronto, including Moss Park, St. Jamestown, and Regent Park.

The DETOHT comprises 43 health and social service organizations deeply rooted in the community, aiming to provide seamless, inclusive, and high-quality care through an integrated system. The DETOHT’s initiatives focus on priority populations, such as people experiencing homelessness and those with mental health and addiction issues, guided by principles of client and family-centered care, health equity, diversity, transparent collaboration, quality improvement, and responsive leadership.

Position Summary:

The Community Health Worker, Inter-professional Primary Care Team will support patient attachment to primary care services with a focus on serving Indigenous and Black communities. The CHW will work closely with the Nurse Practitioner – IPCT, the central navigation team, and other primary care organizations within the Downtown East Ontario Health Team (DET OHT) to deliver integrated services, with an emphasis on health stabilization and fostering strong, ongoing attachment to primary care.

The CHW will provide comprehensive outreach and case management to a diverse population facing significant barriers to healthcare, assisting patients in effectively navigating the healthcare system, promoting health, and addressing social determinants of health. The CHW uses a harm reduction, health promotion, community engagement, community development, social justice, trauma-informed and anti-racist / anti-oppressive (ARAO) strategies.

This is a Full-time Contract position, until March 31, 2028, with Benefits plus opportunity to participate in HOOPP (Healthcare of Ontario Pension Plan).

This is a bargaining unit position with OPSEU Local 5115.

Responsibilities Include
Individual Support – System Navigation:

Provide targeted onsite/offsite support to individuals meeting the criteria for service;
Provide information and referrals, education and service coordination to clients;
Utilize low threshold and client centred strategies to assist clients to gain access to services and supports, onsite at RPCHC and externally;
Engage in assessment and collaboratively developing intervention plans with clients;
Provide crisis intervention in relevant situations and make referrals to other agencies if necessary;
Advocate on behalf of clients on an individual basis;
Monitor and evaluate supports on an ongoing basis
Complete appropriate documentation
Provide accompaniments as needed
Outreach, health promotion and education:

Support implementation of satellite clinics within DET OHT identified locations;
Conduct street-based outreach and outreach to encampments, drop-ins and other centres/agencies used regularly by people who face significant financial challenges, housing insecurity, and other forms of marginalization;
Collaborate with the multidisciplinary team to assist with quality improvement and development of creative strategies to meet client needs;
Provide health information in an accessible, appropriate ways to community and clients;
Community Capacity Building:

Work collaboratively with community partners to support and address client’s issue around health, poverty and homelessness;
Collaborate with team members, agency partners, clients, and communities to plan, implement, and deliver low-barrier, client-centered and coordinated care;
Develop, plan, implement and evaluate community-based initiatives by applying health promotion strategies such as community mobilization, advocacy, healthy public policy and health education;
Qualifications and Skills Required
Post-secondary education in social work, community work, health promotion, adult education and/or equivalent in life experience
3 – 5 years proven case management and system navigation experience with people experiencing homelessness and systemic barriers.
Demonstrated understanding of working within and anti-oppression framework
Understanding of impacts of systemic oppression on marginalized individuals and communities
Experience providing direct support to individuals from Indigenous and Black communities including those who experience homelessness, substance use, mental health issues, and poverty
Proven experience engaging in outreach and low threshold models of engagement and support
Knowledge of and demonstrated ability to apply harm reduction and trauma informed principles and practices
Extensive experience in conflict resolution and crisis intervention
Ability to function as a member of a multi-disciplinary team.
Proficiency in the use of computer technology and various software applications.
Strong oral and written communication skills
Demonstrated interpersonal, decision making, critical thinking and organizational skills
Commitment to Anti-Racist, Anti-Oppressive strategies
Must be drug and *** positive.
Knowledge of the Regent Park and Moss Park communities
Must be fully vaccinated for COVID 19 for the protection of clients and staff
