Role Description

This is a full-time, on-site position located in Sylamr, CA. The Community Integration Specialist (CIS) position works as a one-to-one direct care provider responsible for providing integration training. Community Integration Specialists work in the community with individuals who have been diagnosed with a developmental disability in conjunction with forensic, psychiatric, medical, behavioral complications or other risk factors. Integration Specialists are responsible for the overall safety and care of these individuals and must be capable of addressing severe behavioral issues, addressing toileting issues, dealing with crises (mental health and behavioral).

Responsibilities/Requirements:

· Ensure the safety of clients.

· Assist clients as necessary.

· Addressing maladaptive behaviors.

· Completing daily notes and taking data.

· Ability to summarize events and provide a written narrative report on daily activities and notable events. Share notes, summaries and data with parents/guardians/caregivers.

· Thorough documentation of events and reportable behaviors.

· Follow all Health Insurance Portability Accountability Act (HIPAA), mandated reporting and regional center requirements.

· Follow special incident report (SIR) requirements and reporting procedures.

o Completing SIRs when needed

o Submitting SIRs in a timely manner

· Meet/drop off clients at their homes or various pick-up locations (worksites may vary)

· Provide behavioral interventions and intervene when physical behaviors occur.

· Assist clients with toileting needs.

· Follow, administer and assist with assigned curriculum.

· Engage in physical fitness activities.

· Engage in physical safety interventions.

· Remain within arm’s length, eyesight or ear shot of clients depending on the situation with arm’s length being the default distance.

· Act as a role model for the clients we serve this includes modeling ideal behavior while working such as:

o Making healthy food choices

o Following the dress code

o Engaging in appropriate behaviors

o Avoiding inappropriate behavior such as cursing, yelling, or inappropriate remarks especially in front of the individuals we serve

· Follow directions regarding client restrictions such as dietary, allergy, medical or other directions that must be followed.

· Follow the guidance of supervisors and clinical team.

· Ability to pass a background check (must be free from any misdemeanor or felony convictions and any serious/frequent traffic violations before and during employment)

Qualifications

· High School Diploma

· Current and valid driver's license

· Ability to pass a DOJ, FBI and driving history background check (must be free from any misdemeanor or felony convictions and any serious/frequent traffic violations)

· Ability to follow mandated reporting requirements.

· Negative TB Test

· The ability to pick up and drop off clients.

· The ability to work outside in possible heat, cold, rain and other weather conditions or engage in physical activities when accompanying or covering staff.

· Capability to complete Crises Intervention and Prevention training with advance physical skills.

· Meet regional center vendor requirements: no criminal or serious traffic violations, pass a TB test, receive and abide by CPR/1st aid, HIPAA, bloodborne pathogens, crises prevention/physical intervention training and mandated reporting trainings.

· Safe and reliable form of transportation for pick-up and drop-off of clients when needed. Including all necessary insurance, registration and necessary items for the safe and legal operation of a motor vehicle on public roadways.

Preferred Qualifications

· 2+ years of experience with individuals with developmental disabilities.

· Bachelor’s degree or higher in a related field (e.g. psychology or sociology)

· Knowledge of clinical interventions and data recording.

· Knowledge of developmental disabilities.

· Formal training and knowledge of developmental disabilities and current and acceptable practices.

· Formal training and knowledge of mental health and current and acceptable practices.

· Experience with psychiatric diagnoses

· Experience providing rehabilitation services to individuals who were previously incarcerated.

· Crises Prevention and Intervention (CPI) training or equivalent de-escalation training.

Hours

· Ability to respond to multiple pickup locations by 8:00 am.

· Monday – Friday; 8:00 am – 5:00 pm

☒Kneeling: Bending legs at knee to come to a rest on knee or knees. (e.g. physical fitness days with clients, engaging in physical interventions)

☒Crouching: Bending the body downward and forward by bending leg and spine. (e.g., physical fitness days with clients, engaging in physical interventions)

☒Reaching: Extending hand(s) and arm(s) in any direction. (e.g., assisting clients, shopping, physical fitness days with clients, engaging in physical interventions)

☒Standing: Remaining upright on the feet, particularly for sustained periods of time. (e.g., while in the community working with clients in areas without sitting)

☒ Walking: Moving about on foot to accomplish tasks, particularly for long distances or moving from one work site to another. (e.g., talking clients into the community to engage in activities such as shopping or walking for physical fitness)

☒ Pushing: Using upper extremities to press against something with steady force in order to thrust forward, downward or outward. (e.g., engaging in physical interventions)

☒Pulling: Using upper extremities to exert force in order to draw, drag, haul or tug objects in a sustained motion. (e.g., engaging in physical interventions)

☒Lifting: Raising objects from a lower to a higher position or moving objects horizontally from position to-position. This factor is important if it occurs to a considerable degree and requires substantial use of the upper extremities and back muscles. (e.g., loading items into the van, engaging in physical interventions)

☒Fingering: Picking, pinching, typing or otherwise working, primarily with fingers rather than with whole hand or arm as in handling. (e.g., assisting clients with activities and learning activities that include improving fine and gross motor skills)

☒Grasping: Applying pressure to an object with the fingers and palm. (e.g., assisting clients with activities and learning activities that include improving fine and gross motor skills)

☒Talking: Expressing or exchanging ideas by means of the spoken word; those activities where detailed or important spoken instructions must be conveyed to other workers accurately, loudly, or quickly. (e.g., everyday to provide instructions to clients)

☒Hearing: Perceiving the nature of sounds at normal speaking levels with or without correction, and having the ability to receive detailed information through oral communication, and making fine discrimination in sound. (e.g., everyday to interact with clients and teach communication skills)

The Physical Requirements of this Position:

☒Medium work: Exerting up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects.

The Visual Acuity Requirements including Color, Depth Perception, and Field of Vision:

☒ The worker is required to have close visual acuity to perform an activity such as: preparing and analyzing data and figures; transcribing; viewing a computer terminal; extensive reading; visual inspection involving small defects, small parts, and/or operation of machines (including inspection); using measurement devices; and/or assembly or fabrication of parts at distances close to the eyes.

☒The worker is required to have visual acuity to operate motor vehicles or heavy equipment.

☒The worker is required to have visual acuity to determine the accuracy, neatness, and thoroughness of the work assigned (i.e., custodial, food services, general labor, etc.) or to make general observations of facilities or structures (i.e., security guard, inspection, etc.).

Pay Range: $23-$29 an hour.
