Position Summary
Reporting to the Manager, Community Partnerships and Engagement the Community Partnerships and Engagement Specialist will act as a key liaison with external and internal stakeholders to foster a culture of collaboration between the organization and local communities, helping to ensure the use of a health equity approach to tailor Mosaic PCN programs and services for the highest needs population in our geographic area. The Community Partnerships and Engagement Specialist is a mature professional responsible for supporting the delivery of an inclusive community engagement strategy and activities that will result in improved primary care access, contribute to culturally safe and appropriate care, and have positive patient outcomes.
The Community Partnerships and Engagement Specialist is a fully competent collaborator and relationship builder who is motivated to make a difference. With experience designing, initiating, and delivering relevant community engagement initiatives that offer value to stakeholders, the Community Partnerships and Engagement Specialist supports community partners and individual community members while also working closely with internal Mosaic teams to address organizational strategy and priorities. The ideal candidate is familiar with system change and social impact measurement.
This position also provides guidance, mentorship and support to the Community Partnerships and Engagement Coordinator.
Key Responsibilities
Strategic Planning & Budgeting
• Support the creation, execution and evaluation of MPCN community partnerships and engagement strategy.
• Identify emerging community challenges related to well-being, and recommend appropriate engagement approached to advise internal business units and leaders in the organization on the opportunities they present.
• In conjunction with the Manager, Community Partnerships and Engagement, create and maintains policies, procedures and guidelines related to community partnerships.
• Manages budget within the scope of work to ensure alignment with departmental budget alongside the manager.
• Create strategic plans for large-scale community partnerships and projects, manage the ongoing delivery of plans, working collaboratively with internal and external stakeholders.
• Work with internal Contracts Management team on the development of legal partnership agreements.
• Compliance and performance reporting of community partners.
• Remain informed of population trends, programs, and community partner needs, sharing relevant information as appropriate.
• Assists the CP&E Manager, as required, on business case and/or grant development applications.
• Ability to navigate and execute organizational and/or departmental changes with little notice and within tight deadlines.
Community Relations & Communications
• With the support of the communications department, develop two-way channels of communication with community groups to gather and reflect feedback - write and edit content, including articles for newsletters, presentations, brochures, posters, event materials, etc. consistent with the MPCN brand, direction, and strategy.
• Assist in the development and delivery of inclusion-related content and programming for internal and external stakeholders.
• Collaborate with various MPCN portfolios to share community-related insights and information that supports Mosaic patients.
• Facilitate meetings, engagement sessions and workshops as required with community partners and community members, as well as internal teams where relevant.
• Build mutually beneficial relationships with local agencies, community associations and organizations representing underserved populations to identify barriers to accessing primary care.
• Represent Mosaic in a professional manor at community events and gatherings, as well as committees and roundtables with a focus on seeking out opportunities for collaboration.
• Serve as point of contact for community partner inquiries.
• Work with the CP&E Manager to resolve any community partner complaints and develop a plan forward.
• Serve as point of contact for MPCN to community partners for program outreach and/or studies.
• Provide support and SME expertise on provincial and/or zonal change initiatives that may impact community partners and relationships.
Mentorship
• Provides day to day guidance, support and mentorship to the Community Engagement Coordinator to track, measure and evaluate departmental deliverables, organize logistics and respond to inquiries.
• Represent and act on behalf of MPCN for Calgary Zone collaborative work, including the creation and implementation of zone-wide projects as well as sharing of MPCN findings.
• Maintain alignment with Alberta Health as the funding body for Primary Care Networks as well as the newly formed Primary Care Organizations (details TBD).
Continuous Quality Improvement, Reporting and Measurement
• Identify opportunities for measurement and tracking through a social impact and outcomes lens.
• Proactively track activities of the department according to agreed-upon metrics.
• Implements processes and the use of new tools to evaluate and demonstrate the accomplishment of outcomes.
• Monitor, track, develop, and report on operational and program performance measures and metrics.
• Monitor and track population trends and provide recommendations for program development to meet population demands.
• Work with the Member Services team and Clinical team to promote community partner services and programs for patients.
• Work with the Reporting, Evaluation and Data Integration team and other internal or external stakeholders in the review and updating of community reports and population health reports.
• Performs research and evaluates ongoing development of policies, practices, and procedures.
• Develops, reports and performs moderate to advanced analysis and research including observations and related analysis to provide subject matter responses and assist in driving program improvements to support organizational goals as required.
Qualifications & Requirements
Education & Experience
• Degree in communication, public relations, or community development plus two (2) years' experience, demonstrating past success in developing and executing strategic community engagement plans (or relevant diploma plus four (4) years of experience).
• Partnership building and management experience as well as formal education in public participation is preferred.
• A combination of education and experience will be considered.
Knowledge, Skills and Abilities
• Strategic thinker with the ability to diplomatically ask questions, challenge and suggest alternative strategies and approaches.
• Strong communication and facilitation skills.
• Passionate about fostering meaningful connections between community and organizations.
• Fully competent knowledge and understanding of engagement concepts, strategies and technical skills required in planning, execution and evaluation of engagement plans for internal and external audiences.
• Knowledge of current trends in engagement planning and tactics, including in-person and virtual engagement sessions and other channels.
• Ability to analyze information, problem-solve and make suggestions as to next steps.
• Intermediate-advanced knowledge of Microsoft Office Suite to produce plans, reports, and materials as well as the ability to conduct appropriate online research.
• Experience cultivating and leveraging strategic partnerships.
• Experience in conflict resolution and negotiations.
• Change management experience.
Assets
• Experience in a healthcare environment with an understanding of patient-centered care philosophy or experience with community engagement in a quasi-governmental institution is an asset.
• Member of the International Association of Public Participation (IAP2).
Working Conditions
• Occasional evening and weekend hours required.
• Ability to travel on public transportation or a valid driver's license with access to own car in good repair is required.
• Occasional standing and light lifting as required when setting up for and attending community events.
Working Environment: This is a hybrid position, working in an open office environment and form home as agreed upon with the manager.
