Ontario Health atHome is looking for an experienced leader in Patient Services to be accountable for leading patient services teams and collaborating with partners in primary care, community services and acute care to deliver an exceptional integrated patient and caregiver experience, high quality care/services and ensure the most optimal outcomes for patients and families we serve.

Salary: $135,+01.37 to $162,272.+4

What will you do?
Provides overall leadership and accountability for the delivery of high quality care to patients and their families receiving care in the community and transitioning from and to other care settings including hospitals, long-term care homes, etc.
Collaborates with service providers, primary care, community services, and acute care partners to ensure the delivery of quality of care.
Responsible for the development, management and oversight the financial resources/budget of the area ensuring patient care goals are met within allocated resources.
Provides strategic leadership develop, implements, and evaluates improvement plans to advance integrated home and community care services at the sub-region.
Develops trust/credibility and strong partnerships with a broad range of internal and external stakeholders including primary care, community care and acute care leaders to create innovative, strategic partnership opportunities to advance home and community care as part of an integrated service delivery system.
Represents and brings the expertise of Home and Community Care at external and internal planning tables/committees, as required.
Identifies, plans, directs, organizes and evaluates health care services for integrated patient focused home and community care programs ensuring services are provided in compliance with legislation, MOHLTC provincial policies, procedures and overall mission, vision, values, philosophy and goals of the organization.
Engages in strategic, operational and financial planning of the department and programs ensuring integration within the organization and across organizations.
Champion’s patient and caregiver engagement and co-design of program improvements, ensuring the patient/caregiver voice and experience shape all program improvements.
Provides necessary coaching and leadership to patient services managers and teams to best ensure successful outcomes.
Responsible for human resources planning, staffing and performance to ensure efficient and effective operations.
Fosters and maintains high staff engagement and positive employee relations

What must you have?
Must be a Regulated Health Professional with valid certification in a professional college in the province of Ontario.
Post-secondary/Master’s degree in a health discipline, health administration, public administration or other related field.
5-10 years of progressive work experience managing within a health care environment.
Strategic and conceptual thinker with a systems orientation with demonstrated ability to implement new programs and develop/lead high performing teams.
Demonstrated ability to develop, maintain and evolve strong relationships, collaborations and partnerships with internal and external stakeholders.
Strong knowledge of quality improvement tools, strategies and supporting systems.
Strong understanding of integrated service delivery models, emerging health care issues and future opportunities for the healthcare environment in order to ensure the development of effective patient service strategies and programs.
Demonstrated effective experience with financial planning and management processes.
Highly developed change management skills to lead an evolving patient service delivery role and the advancement of new integrated models of care.
Superior verbal, oral and written communication and conflict resolution skills to work with staff, patients and partners at the executive level in resolving complex problems, complaints and appeals.
Demonstrated superior verbal and written communication and presentation skills.
