Experienced Disability Support Worker Needed – Western Melbourne (Various Sites)
Responsibilities and Duties
Support clients to participate in age-appropriate activities and tasks, including employment, in-home support, and community access.
Assist clients to access activities and outings within the community.
Provide direct care assistance to each client in accordance with their service plan and as directed.
Work in accordance with medication administration policies, Work Health and Safety (WHS) procedures, and client rights policies, including privacy and confidentiality.
Monitor the safety and well-being of clients and report any concerns to the Manager.
Provide one-on-one or small group training and support in response to individual client needs.
Ensure completion of appropriate documentation. Relevant information on supports provided must be recorded in client notes and communication books as required.
Participate in the developing participant support plan process for individual clients and monitor outcomes.
Work collaboratively with clients, families/carers, case managers, and other professionals to support individual goals.
Assist, in conjunction with other staff, in developing, implementing, and evaluating training and behaviour support programs to achieve identified goals.
Ensure all programs are appropriately maintained, documented, and reviewed.
Assume the role of key worker for designated clients, including preparation of monthly progress reports.
Liaise effectively with other staff to ensure smooth and consistent service delivery.
Report regularly to the Manager.
Contribute actively to a team environment through open communication, participation in staff meetings, planning processes, and policy reviews.
Provide backup support to other staff as required.
Participate in Performance Development meetings to identify ongoing training and development opportunities.
Notify the service as early as possible if you are unable to attend your rostered shifts.
Contribute to case reviews with Employment Consultants and Facilitators.
Service Delivery Expectations
Clients receive appropriate levels of support to promote and maintain independence and participation in age-appropriate activities.
All clients receive a high standard of care in line with their Lifestyle Service Plan (LSP).
Personal care is delivered in a respectful, caring, and supportive manner.
Documentation demonstrates compliance with service procedures and standards.
All organisational policies and procedures are followed.
Records reflect accurate medical and dietary intake as prescribed by health professionals and in line with organisational policies.
Client files are maintained, updated, and reviewed, with relevant health care and behaviour support plans in place.
Lifestyle Service Plans and annual reviews are completed and approved by the Manager.
Daily documentation (e.g. diaries and activity records) is completed accurately.
Maintain a high level of communication with the Manager, including reporting potential risks to clients or the organisation.
Complete required training, including induction, WHS, manual handling, medication, and infection control.
Ensure all documentation is completed and maintained in accordance with procedures.
Provide sufficient notice if unable to work rostered shifts.
Adhere to the Employee Code of Conduct at all times.
