Medseek Clinics: Exciting Roles for Health Coach & Health Improvement Practitioner
Are you a passionate registered health professional ready to make a real impact? Medseek Clinics is looking for dedicated individuals to join our team in two essential roles: Health Coach and Health Improvement Practitioner (HIP). These positions are crucial to our commitment to holistic, integrated patient care and community health.
Position 1: Health Coach
As a Health Coach, you will:
Deliver health coaching aligned with the Health Coach curriculum from the Centre of Excellence in Primary Care and its NZ adaptation.
Collaborate with GPs and practice nurses to support patient self-management and prevent/manage long-term conditions.
Facilitate wellness courses and document all patient interactions accurately in our clinic systems.
Ensure cultural harmony, connecting patients to health resources and community groups like local churches and marae.
Work with patients on emotional support due to chronic illness, enhancing their well-being over time.
Position 2: Health Improvement Practitioner (HIP)
In this role, you will:
Implement the Behavioural Health Consultancy model tailored for New Zealand in individual, group, and whānau sessions.
Offer brief, evidence-based interventions (15-30 minutes) for a range of issues covering mental and physical health across all ages.
Help patients identify and achieve their goals in line with their care plans, with a focus on prevention, acute care, and long-term condition management.
Demonstrate culturally safe practices and provide accessible interventions to our enrolled population.
Record all client interactions and ensure compliance with documentation standards in patient management systems.
