About the Role:
We are looking for a full-time Clinical Care Coordinator who will be responsible for coordinating clinical services in order to support clients in their own home. The successful candidate will work as a team to provide efficient delivery of services and achieve the best possible outcome for our clients.
Main Duties Will Include:
Coordination of care for new and existing clients of the SA Health Community Care Program
Acceptance of referrals including the Metropolitan Referral Unit (MRU) and ensuring all client information is available
