Full job description
We are seeking a dedicated and compassionate Home Care Nurse to provide high-quality nursing care to patients in their homes under the direction of our hospital’s home care program. The ideal candidate will deliver patient-centered care, coordinate with multidisciplinary teams, and promote recovery and quality of life for patients outside the hospital setting.

Key Responsibilities:

Assess, plan, implement, and evaluate individualized nursing care for home-bound patients.
Administer medications, wound care, injections, and other nursing interventions as per physician's orders.
Monitor vital signs, medical conditions, and recovery progress.
Educate patients and families on disease management, medication adherence, and post-discharge care.
Maintain accurate documentation and patient records according to hospital and legal standards.
Coordinate with physicians, therapists, and social workers for comprehensive care planning.
Ensure infection control practices are followed in the home environment.
Provide emotional support to patients and caregivers, promoting independence and dignity.
Respond to emergencies and escalate concerns promptly.
Adhere to hospital policies, procedures, and regulatory standards.
Qualifications:

Education: Diploma/BSc in Nursing from an accredited institution.
License: Valid [State/National] Nursing License (e.g., RN).
Experience: Minimum 1–2 years of clinical experience, preferably in home care, community health, or medical-surgical nursing.
Skills:
Strong clinical and assessment skills
Excellent communication and interpersonal abilities
Ability to work independently with minimal supervision
Time management and organizational skills
Proficiency with electronic health records (EHR)
Working Conditions:

Regular travel to patient homes within the service area.
Flexible scheduling including evenings or weekends as needed.
May involve exposure to various home environments and physical activities.
Preferred Qualifications:

Certification in Home Health Nursing or Geriatric Care (optional but advantageous)
Experience in palliative care or chronic disease management
Fluency in multiple languages is a plus

Pay: From ₹32,000.00 per month
