• Maintain professional standards of competence in line with current best nursing practices.
• Evaluate patient needs and ensure that all care requirements are met in accordance with the care plan, promoting patient safety, privacy, dignity and confidentiality at all times.
• Accurately record all relevant information relating to a patient's care or treatment, as per the care delivery guidelines.
• Communicate effectively with referring clinicians in a timely and professional manner.
• Collaborate closely with other healthcare professionals as necessary, participating in quality assurance programs and adhering to established policies and procedures.
• Deliver patient care post-discharge at the patient's residence, ensuring continuity of care.
