Provides care and respect to patients in a clean and safe environment
Demonstrates effective communication with other members of the health care team and promotes effective team functioning
Participates in IDT
May have access to IV solutions, authorized medications and supplies as needed for patient care responsibilities; only utilizes items which are on the assigned patients Hospice Aide Care Plan
Collects, records, and reports accurate patient information
Follows policy +3 for timely documentation submission
Delivers basic care to patients as directed in the plan of care
Assists other associates to maintain productivity
Demonstrates dependability, reliability and flexibility
Uses are cares for equipment and supplies economically
Completes assigned tasks in a timely manner
Provides service to patients and families with sensitivity and respect for their needs, expectations, age, cultural and individual differences
Responds to call light in a timely manner, when assigned to work in the Hospice House
Potential for exposure to bio hazardous waste, and known or unknown diseases
Flexibility in work schedule to cover other areas of Hospice services (field, inpatient unit, respite/crisis care)

POSITION SUMMARY:
