Job Title: Hospital Pharmacist

Overview: As a Hospital Pharmacist, you will play a critical role in ensuring the safe and effective use of medications within a healthcare facility. Working as part of a multidisciplinary healthcare team, you will be responsible for dispensing medications, monitoring drug therapy, and providing clinical pharmacy services to optimize patient outcomes. Your expertise in pharmacology, medication management, and patient counseling will contribute to the delivery of high-quality healthcare services and patient safety.

Responsibilities:
• Medication Dispensing: Dispense medications accurately and efficiently, ensuring proper labeling, dosage, and instructions for use, in accordance with physician orders and regulatory requirements.
• Medication Management: Manage medication inventory, including procurement, storage, and distribution, to ensure adequate supply and availability of medications for patient care.
• Medication Therapy Monitoring: Monitor patients' medication therapy for safety, efficacy, and adherence to treatment guidelines, identifying and addressing drug-related problems and adverse drug reactions as needed.
• Clinical Consultation: Provide clinical pharmacy consultation to healthcare providers, including physicians, nurses, and other allied health professionals, regarding drug selection, dosing, interactions, and therapeutic alternatives.
• Patient Counseling: Counsel patients and their caregivers on medication use, potential side effects, drug interactions, and proper administration techniques to promote medication adherence and optimize therapeutic outcomes.
• Medication Reconciliation: Perform medication reconciliation processes, reconciling discrepancies in medication lists across transitions of care to minimize medication errors and adverse events.
• Drug Information: Provide accurate and timely drug information to healthcare professionals and patients, including evidence-based recommendations, drug monographs, and formulary management.
• Clinical Documentation: Document clinical pharmacy interventions, patient counseling sessions, and medication-related activities accurately and comprehensively in the electronic health record (EHR) system.
• Quality Assurance: Participate in quality assurance activities, such as medication safety audits, medication use evaluations, and adverse drug event reporting, to identify opportunities for improvement and enhance patient safety.
• Education and Training: Participate in educational activities and training programs for healthcare staff, patients, and the community on topics related to medication safety, pharmacotherapy, and medication management.

Requirements:
• Education: Doctor of Pharmacy (Pharm.D.) degree from an accredited pharmacy program.
• Licensure: Current licensure as a pharmacist in good standing, with eligibility for licensure in the applicable jurisdiction.
• Clinical Skills: Proficiency in pharmacotherapy, pharmacokinetics, and pharmacodynamics, with a strong foundation in clinical pharmacy practice.
• Communication Skills: Excellent verbal and written communication skills, with the ability to communicate effectively with patients, healthcare professionals, and other stakeholders.
• Teamwork: Ability to work collaboratively as part of a multidisciplinary healthcare team, contributing expertise and insights to patient care decisions and quality improvement initiatives.
