Job Description
Amergis is hiring a RN Care Coordiantor to support the transition of care from facility based care to community based care. The Nurse Care Coordinator may work with patients being discharged from a hospital or skilled nursing facility.
Responsibilities
• Reaches out to facility contact to obtain discharge information and associated plans for patients to support their post-transition care
• Develops a plan with the patient to meet the identified post discharge needs.The plan may include home visits by the RN Coordinator, accompanying patients to their first visits, or scheduled phone calls.
• Outreaches and responds to the patient and/or care giver and introduces self, performs screening and interviews patient to assess needs
• Interfaces with care teams to ensure the application of care coordination protocols for screening assessment and care planning
• Reports any incidents of suspected abuse, neglect, or exploitation to supervisor immediately upon discovery
• Reports feedback to medical home teams according to department policy and procedure
Minimum Requirements
• Bachelor's Degree of Science in Nursing (BSN) from an accredited college or university
• Licensed as a Registered Professional Nurse in the State of Illinois
• 2 years of recent work experience with responsibilities for care coordination across multiple healthcare settings and providers
• 2 years of recent experience using electronic medical records, care management systems, or scheduling software
• 1 year of recent experience in utilization management or case management
• Valid Driver's license and mandatory vehicle insurance or other dependable means of transportation
