• Offer crisis intervention and support during difficult situations, such as terminal illness, trauma, or sudden changes in health status.
• Collaborate with the healthcare team, including doctors, nurses, and therapists, to develop and implement comprehensive care plans.
• Coordinate with external agencies, such as home health services, hospice care, and community organizations, to ensure that patients receive the necessary resources and support.
• Facilitate communication between patients, families, and healthcare providers to ensure that the patient's needs and preferences are met.
