Key Responsibilities

Telephonic and face-to-face assessment, planning, implementation, and coordination of care management activities with members.
">
Proactive course of action to address issues presented and facilitate short- and long-term outcomes.

Comprehensive assessments of members' needs and recommendations for case resolution.
">
Clinical judgment to incorporate strategies designed to reduce risk factors and address complex health and social indicators.
">
Assessments taking into account information from various sources to address all conditions.
">
">
">
Requirements
">
">
Must reside in Miami-Dade, FL or a surrounding county.
">
Bilingual in Spanish.
">
Active current unrestricted RN licensure in the state of Florida.
">
2+ years of experience with pediatric patient care as an RN.
">
Willing and able to travel 10-20% of the time.
">
Reliable transportation required.
">
2+ years' experience using personal computers and MS Office Suite applications.
">
">
">
Preferred Qualifications
">
">
Experience managing pediatric behavioral health conditions.
">
Case management experience.
">
Certified Care Manager (CCM) Certification.
">
Managed care or utilization review experience.

Salary Range

$54,0+5.60 - $116,760.80.
