We are seeking an experienced Physical Therapist Assistant to join our team at the facility. As the Physical Therapist Assistant, you will be responsible for providing Physical Therapy treatments to facilitate increased independence and functioning under the supervision of a Licensed Physical Therapist.

Responsibilities:

Provide skilled physical therapy interventions, including therapeutic exercise, manual therapy, and modalities
Communicate with interdisciplinary team members, including physicians, nurses, Physical Therapists, Occupational Therapists, and Speech Therapists, to coordinate care and ensure optimal outcomes for residents

Educate residents and family members on physical therapy goals, treatment plans, and home exercise programs
Maintain accurate and timely documentation of physical therapy services provided
Participate in team meetings, in-service education, and quality improvement initiatives
Maintain current knowledge of physical therapy practice and relevant regulations
Qualifications:

Current license to practice as a Physical Therapist Assistant in the state of California

Minimum of 2 years of experience in a healthcare setting, preferably in a skilled nursing facility
Knowledge of current physical therapy practice standards and regulations
